Agency Overview
Since opening its doors in 2012, MISI Community Solutions helped individuals navigate the evolving world of Medi-Cal Managed Care health plans and Community Supports. MISI team works alongside its clients to find the best, medically appropriate care options in the least restrictive setting.
Programs:
1 - Enhanced Care Management (ECM), where a dedicated Lead Care Manager coordinates healthcare, social services, and directs you to the community support.
2 - Housing Assistance program provides help with Housing Navigation, Housing Deposits, Resourses Assistance, and Sustainability.
3 - Doula Service - provide compassionate, culturally sensitive support to pregnant individuals before, during, and after childbirth.
4 - Community Health Worker (CHW) - connects people to health services, resources, and support. They help individuals navigate the healthcare system, provide health education, and offer trusted, culturally sensitive gui
